What gets less attention is what the safety monitoring systems are quietly recording. Two sets of data published in mid-2026 — one from the FDA's adverse event database, one from the European pharmacovigilance system — offer the most detailed look yet at two specific safety questions that patients and prescribers should understand.


The gallbladder question: Do GLP-1s increase the risk of biliary events like gallstones and cholecystitis, and are some drugs riskier than others?


The thyroid question: Does the black-box warning about thyroid C-cell tumors, based on rodent studies, translate to real risk in humans?


The short answers: yes to the gallbladder concern (with meaningful differences between drugs), and not conclusively on thyroid cancer — but the monitoring continues.

A May 2026 study published in a peer-reviewed journal analyzed the FDA Adverse Event Reporting System (FAERS) — the agency's spontaneous reporting database — to look specifically at biliary adverse events across different GLP-1 receptor agonists (signal-pubmed-8b030ab5). The events included gallstones (cholelithiasis), gallbladder inflammation (cholecystitis), biliary colic, and bile duct obstruction.


The key finding was that biliary adverse event rates are not uniform across the drug class. Some agents showed markedly higher reporting ratios than others. This matters because it means prescribers may be able to factor gallbladder risk into drug selection — a level of precision that generic "GLP-1" safety counseling does not capture.


Why GLP-1s Affect the Gallbladder


The mechanism is biologically plausible and involves two independent pathways:


Pharmacologic effect. GLP-1 receptors are expressed on the gallbladder and biliary tract. When activated, they slow gallbladder motility — essentially, the gallbladder contracts less frequently and empties less completely. This creates conditions favorable to bile stasis, which promotes gallstone formation.


Weight loss effect. Rapid weight loss — the kind GLP-1s produce — mobilizes cholesterol from adipose tissue into the bile, altering its composition. Cholesterol-rich bile is more likely to crystallize into gallstones. The combination of pharmacologic gallbladder hypomotility plus rapid cholesterol mobilization creates a compound risk that neither factor alone would produce.


This is the same reason bariatric surgery patients have elevated gallstone risk, and why many surgeons recommend prophylactic cholecystectomy during gastric bypass procedures.


Risk Factors for GLP-1-Associated Biliary Events


- Pre-existing gallbladder disease or known gallstones
- Rapid weight loss (greater than 1.5 kg per week)
- Female sex (higher baseline gallstone risk)
- Age over 40
- Prior pregnancy (multiple pregnancies increase risk)
- Certain lipid profiles (high cholesterol, low HDL)


What to Watch For


Right upper quadrant pain — particularly after fatty meals — is the classic gallbladder warning sign. Postprandial abdominal discomfort, nausea with fatty foods, and jaundice (yellowing of the skin or eyes) all warrant evaluation. Ultrasound is the first-line diagnostic test.

The study's findings contribute to an ongoing debate rather than settling it. Current clinical consensus: the totality of evidence does not support a causal association between GLP-1s and thyroid cancer in humans. This aligns with the FDA's own June 2026 safety resolution on related neuropsychiatric signals — the agency reviews safety questions thoroughly and updates guidance when evidence dictates.


What This Means for Patients
